The skin is not only a protector and an important organ of the human body, it is extremely flexible and generally adapts smoothly to each change of the body. However, significant weight fluctuations, pregnancy and the natural ageing process can all cause the abdominal muscles and the skin on the tummy to become weak and loose. When skin and tissue are too stretched out they cannot naturally return to their original size and become resistant to diet or exercise.
Yet having a slim, toned tummy is important to lots of us when it comes to feeling good about ourselves. It can increase our self-esteem and self-confidence, allow us to wear swimwear and more fitted clothing.
The abdominoplasty procedure, also known as tummy tuck, is a widely acknowledged and effective technique to remove excess skin and tissues in the abdominal wall area, while restoring weakened or separated muscles creating a firmer and flatter abdomen. Prior to this procedure frequently a liposuction is performed to remove “love handles” and other fat areas.
Tummy tuck is suitable for men and women.
Similar to a caesarean-section, a surgical incision is made in the bikini area of the lower abdomen. The size of the incision depends on the amount of skin and fat to be excised: the less loose skin, the shorter the incision/scar. Regardless of length incision will be placed so that it can be concealed under clothing, underwear and swimwear.
In a full tummy tuck procedure an additional incision is made to remove the belly button, then muscles are pulled back together and stitched in place. Excess fat is removed and the belly button repositioned. Excess skin is then removed. The muscles are tightened in the midline where the tissue had been stretched. Permanent stitches are often used. The operation is performed under general anaesthetic.
Recovery may take up to 6 weeks and no physical activities are allowed. Tailored compression garment needs to be worn to minimize swelling and to support the abdomen as it heals. Resume of work can take place after 2-3 weeks.

Liposuction
A combination of a healthy diet and physical exercise can reshape the ideal body you want, but unfortunately there still may be some areas that are resistant to traditional weight loss methods. A lot of people get annoyed by the stubborn pockets of fat that are usually located in typical problem areas. This may cause a negative well-being. Liposuction is the process of removing unwanted fat of common treatment areas like “saddlebags” by using small cannulas introduced through small incisions. This kind of surgery should be not regarded as a method for weight loss, but as a technique for reshaping, forming and harmonising the body proportions.
The fat cells that are removed by liposuction can never return, however, if the patient begins to increase his calorie intake, then remaining fat cells could increase in a disproportionate way.
Typical areas of the body for women that can be treated with liposuction are: buttocks, hips, abdomen, legs, facial/neck fat.
For men, liposuction can be used to target: abdomen, hips or “love handles” and breast (see also gynecomastia).
Before the procedure begins a liquid solution (tumescent solution) will be injected into the area being treated to remove the fat more easily. If it is only a small area being treated then local anaesthetic may be used. With bigger areas, it is required to have it done under general anaesthetic.
Once the skin is prepared, a small cut is made in the area to be treated and the fat cells can be suctioned. After the operation, the treated area will be stitched and you need to wear a compression garment for 6 weeks enabling the skin to adjust to the new body shape. This healing process can be supported by applying special creams.

Gynaecomastia
Enlargement of the breast in men may not only be caused inherently by weight gain but also by side-effects of certain pharmaceuticals. Mostly this concerns fatty tissue and rarely glandular tissue. Although the change is normally benign and no procedure from a health point of view is required, a prominent male breast may significantly affect the self-esteem of these men.
The female breast shape has such an impact that men not only avoid wearing tight clothes, but also going to swimming pools and saunas.
If the shape of the breast cannot be corrected by neither exercise nor diet gynaecomastia surgery offers the possibility to remove these female-like breasts. Before planning a treatment a hormonal test should be carried out. Furthermore, a urological examination should take place to rule out any serious diseases as the cause.
Depending on the extent of your gynaecomastia either liposuction is performed or excess glandular tissue will be removed by making a small incision around the areola. The surgery can be performed under local or general anaesthetic. After surgery an elastic pressure garment should be worn for 6 weeks to optimize the result. It is recommended to avoid sports. Work can be resumed after 1-2 weeks.

Arm lift
The numbers of women with concerns about their ‘bingo wings’ are steadily increasing. Due to ageing and weight gain/loss the skin on the upper arms becomes loose and starts sagging; loss of skin elasticity and reduction of tissue volume. The swing of the excess skin can often be so uncomfortable that women avoid wearing sleeveless outfits and have problems buying ready-to-wear clothing. Brachioplasty, also known as arm lift, is a surgical procedure removing excess skin and tissue. Main goal of this treatment is to regain toned and sculpted upper arms aligning perfectly with the body.
Liposuction is also an option for patients with small to moderate amounts of fat and little skin laxity. It is a minimally invasive technique removing unwanted fat deposits through small incisions in the skin. These inconspicuous incisions of a few millimetres are made in the armpit and typically leave no scarring behind.
For patients with significant excess skin the arm lift procedure needs to be carried out under general anaesthetic. To completely remove excess fat and skin an incision in the upper inner arm is made, extending from armpit to elbow. Subsequently, multiple layer closure is performed and a wound dressing is applied. After the surgery, it is required to wear compression garment for a minimum of 6 weeks. This is absolutely necessary to hold the skin in place and allow it to contract resulting in smooth skin appearance and prevention of swellings.
Whether an arm lift surgery should be performed on an outpatient or inpatient basis, this will be discussed with you in person during your consultation. Recovery period usually takes about three to four weeks. During this period the arms should not be elevated. Most patients can resume work after one week, if the job is not very physical and does not require heavy lifting. Despite the most inconspicuous location for a brachioplasty scar, you should be aware that when moving around scarring may be visible.
